Instructions for usernames and passwords for all departments

All users should have received a username and password from your department's supervisor(s). Generally, the format is as follows: utsw + first initial + last name (i.e. John Smith = utswjsmith), and the password is the same as the username but needs to be changed as soon as you log for the first time.

Everyone MUST add their state license and expiration date, national registry license and expiration date, date of birth if its missing, and employee ID# the first time you log in.

Human Participant Protections Education Course

This web-based course presents information about the rights and welfare of human participants in research. The two-hour tutorial is required for those conducting research involving human participants.

You MUST print and provide a copy of your completion certificate to your department upon finishing the course.
